INDEPENDENT MEDICAL EVALUATIONS (SECOND OPINION) include a review of Medical Records, Review of 7-Day Pain Diary (if necessary), and Initial Injury/Pain Interview, and Injury/Pain Evaluation and a written statement of Medical Opinion.
